Radiograph of a hysterosalpingogram revealing marked distension of the lower uterine segment and deviation of the uterine cavity toward the left side of the pelvis with the uterus stretched around a very large right sided transmural leiomyoma (fibroid). The right fallopian tube is filling and spilling while the left fallopian tube has proximal tubal occlusion (blockage) without any fill.

One possible problem following removal of a fibroid of this size (to be considered when assessing the risks and benefits of myomectomy) is that the resulting defect in the uterine wall may be quite large and difficult to repair (reclose) in such a way that the normalcy of the uterine cavity is preserved and functional.
