Photograph of the hysteroscopic view of a uterine cavity demonstrating the left tubal ostium (region containing the opening to the left fallopian tube) with a small polypoid structure in the endometrium adjacent to the tubal orifice. Resection in this region is often difficult with the resectoscope loop electrode (24 French) that I commonly use since the space is very narrow (tight), so switching to an operative hysteroscope with smaller diameter flexible graspers or scissors is occasionally required. I try not to leave any identified defects or visible abnormalities within the uterine cavity (even small ones) since they may interfere with reproduction. The role of small defects like this one in terms of implantation is not clear and repair is therefore highly controversial.
